Your Apple Watch has decided your cardio fitness is low. The message is blunt, it arrives with almost no context, and it is easy to read as a verdict on your health. Here is what it actually means, how much of it to believe, and what genuinely moves it.
The short version: the notification means your estimated VO2 max sits in the bottom fifth for your age and sex. It is an estimate built from how your heart responds to walking or running outdoors, not a laboratory measurement, and it is one of the more improvable numbers on your wrist.
What does low cardio fitness mean on Apple Watch?
Cardio fitness is Apple's name for estimated VO2 max: the largest volume of oxygen you can take from the air and use, given in millilitres per kilogram of body weight per minute. Apple states the thresholds openly. For ages 20 to 59, low means the lowest quintile for your sex and age decade, taken from the Fitness Registry and Importance of Exercise National Database. From 60 onwards it switches to absolute cut-offs: 18 ml/kg/min for men and 15 for women, based on figures associated with living independently at older ages.
Low is a statement about where you sit in a population, not a diagnosis about you. Two people with identical numbers can get different messages, because the comparison is always against others of the same sex and age.
How does a watch estimate VO2 max without a lab?
Measuring VO2 max properly means cardiopulmonary exercise testing: a mask, a treadmill or a bike, and rising intensity until oxygen consumption stops climbing. Your watch does something cheaper. It watches how your heart rate responds to a workload it infers from GPS and motion, then projects upward from that submaximal response. You never have to go flat out, but the projection leans on an assumption about your maximum heart rate, which is why heart-rate-limiting medication breaks it so badly.
That is also why the estimate is fussy about conditions. Apple's paper specifies an Outdoor Walk, Outdoor Run or Hiking workout, on ground with less than 5 percent incline or decline, with usable GPS and heart signal, and enough exertion: roughly 30 percent of the way from your resting heart rate to your maximum. Your first qualifying workout will not produce a number. Since watchOS 7 the supported range runs from 14 to 60 ml/kg/min.
So if your watch shows no cardio fitness at all, the usual cause is that you have never done a qualifying outdoor workout, not that you have fallen off the bottom of the scale. In Apple's own data, 78 percent of eligible outdoor workouts produced an estimate, and 93 percent of participants had one within their first ten.

How accurate is the estimate?
Better than its reputation, and worse than a lab. In Apple's validation group of 221 people, the watch estimate averaged 1.4 ml/kg/min above the reference value, with a standard deviation of 4.7. Repeat estimates more than 28 days apart agreed with an intraclass correlation of 0.86, which is higher than the 0.75 Apple cites for submaximal treadmill testing.
The standard deviation is the number worth sitting with. An error band of roughly five ml/kg/min either way is wide enough to move you a whole fitness category, so a single reading near a threshold is not worth reacting to. A trend across a season is.
The setting that most often makes it wrong
If you take a heart-rate-limiting medication, a beta blocker or a calcium channel blocker, and you have not told the Health app, the estimate is not slightly off. It is broken. In Apple's validation cohort the error for those users ran at 11.8 ml/kg/min before the medication was declared, and 1.6 ml/kg/min after. Health Details in the Health app is where that lives, and it is the single highest-value minute you can spend on this number.
Apple lists the other ways the estimate goes low without your fitness changing: carrying significant extra weight such as a backpack or a child, walking on sand, pushing a pram, using a walking aid, plus dehydration, caffeine, extreme heat and a recent move to altitude. An incorrect age, sex or weight in the Health app will quietly bias every estimate you ever receive.
Does a low number actually matter?
The association between cardiorespiratory fitness and mortality is one of the better-replicated findings in the field. In a Cleveland Clinic cohort of 122,007 adults referred for treadmill testing and followed for a median of 8.4 years, the lowest-fitness group carried an adjusted hazard ratio of 5.04 for death from any cause compared with the fittest group. For scale, in the same analysis smoking came in at 1.41, diabetes at 1.40 and coronary artery disease at 1.29. The authors found no upper limit to the benefit of being fitter.
Two caveats, honestly. That was a clinical population being sent for a stress test, not a random sample of healthy adults. And it is observational, so fitness partly stands in for everything else that makes a person able to run on a treadmill. The direction of that finding is solid; the exact multiplier for any one person is not.
How much of it can you actually change?
Not all of it. Apple's review cites genetics as accounting for roughly 50 to 70 percent of the differences in VO2 max between individuals, and 20 to 60 percent of the variation in how much any one person improves with training. That is a real limit on comparing yourself with other people, and no limit at all on comparing yourself with yourself six months ago.
On how to train it, the internet is louder than the evidence. A meta-analysis of 28 controlled trials covering 723 participants found that endurance training raised VO2 max by 4.9 ml/kg/min against untrained controls, and interval training by 5.5. The advantage of intervals over ordinary steady training came to about 1.2 ml/kg/min, a fraction of the gap between training and not training at all.
Which points somewhere unglamorous. Two or three easy aerobic sessions a week, one of them long enough to be dull, plus something harder when you actually have the appetite for it, will move this number. And because the estimate only updates on outdoor walks, runs and hikes, some of those sessions have to happen outside for it to notice.
What to check this week
Start with the inputs, not the training. Open the Health app, confirm your age, sex and weight, then declare any heart-rate-limiting medication under Health Details. After that, do two or three outdoor walks or runs on flat ground, tracked as workouts, at a pace that is clearly work rather than a stroll. Then leave it alone for a month.
It is worth watching what happens around the number as well as to it. A resting heart rate that drifts down over months is the same adaptation, measured every night instead of only on qualifying workouts, and it usually notices first.
