The cleanest real-world answer comes from a study of 4,098 Finnish employees published in JMIR Mental Health. It measured heart rate variability during sleep after ordinary, unplanned drinking, and the effect was strictly dose dependent. An HRV-derived measure of physiological recovery fell by 9.3 percentage units after a low dose, 24.0 after a moderate one and 39.2 after a high one, concentrated in the first three hours of sleep. Those first hours are when most of your overnight recovery happens, and alcohol takes that window first.
A much larger 2026 analysis in PLOS Digital Health, covering 20,968 people and more than five million person-days of wearable data, found the same shape at scale. One drink above a person's own average was associated with a resting heart rate roughly 2.8 bpm higher in women and 2.4 bpm higher in men, HRV roughly 3.8 ms lower in women and 3.3 ms lower in men, shorter sleep, and less physical activity the following day.

Why does one night of drinking hit the numbers so hard?
Alcohol is a sedative going in and a stimulant coming out. A 2026 narrative review in Nutrients describes the pattern clearly: alcohol shortens the time it takes to fall asleep and raises slow wave sleep early in the night while suppressing REM. Then, as the ethanol is metabolised, the sedation gives way to more waking after sleep onset and lower sleep efficiency across the second half of the night.
Your heart follows that curve. Sympathetic activity climbs while the parasympathetic side, the one HRV mostly reflects, gets pushed into the background exactly when it should be doing its work. Does the four to five day claim hold up?
You will see it repeated constantly: one night of drinking sets your recovery back four or five days. We could not find a controlled study supporting that figure for ordinary drinking, so we are not going to repeat it as though it were established.
What the controlled work does show is subtler and more interesting. A 2021 study in Scientific Reports gave 15 healthy volunteers intravenous ethanol and recorded their ECG at baseline, at peak alcohol concentration, and again once alcohol had cleared. The parasympathetic marker returned to baseline as the alcohol left. Heart rate, the corrected QT interval and a sympathetic marker did not fully return. Your autonomic system does not reset the moment your blood alcohol reads zero, but the gap that study describes is measured in hours, not days.
So here is the honest version. A normal evening usually costs you one night, and a heavy one can visibly cost two or three. Anything beyond that is not a general rule, it is a fact about you, and it is knowable, because it is already sitting in your own data.
Why does the same drink cost you more than it costs someone else?
Body size and water distribution explain part of it, which is why the effects in the PLOS Digital Health data were consistently larger in women than in men at the same intake. Age explains more. Disruption was larger in younger adults, with the sharpest gap between people in their twenties and people in their thirties.
The uncomfortable finding is about fitness. In the Finnish sample, regular physical activity did not blunt alcohol's effect on the autonomic nervous system, and younger participants showed stronger effects on heart rate and RMSSD rather than weaker ones. Being fit does not buy you a discount on what alcohol does overnight. It is one of the clearest arguments for why your own baseline beats any published normal range.
How do you make the night cost less?
The PLOS analysis also looked at what went with smaller effects, and three things stood out: drinking earlier in the day, getting more sleep afterwards, and keeping exercise lighter on drinking days. The Nutrients review makes a similar point about timing, noting that finishing alcohol three to four hours before sleep reduces the disruption without removing it.
None of that is a lecture about drinking less. It is a set of levers that change what tomorrow's chart looks like, if you want them. Drink earlier rather than later. Protect the hours of sleep, because losing sleep on top of alcohol stacks two hits into one night. And do not schedule your hardest session for the morning after and then read the result as a verdict on your fitness.
